Less likely to receive nonrecommended antibiotics. When we evaluated patients with no history of UTI, nonwhite race and visits to obstetricians gynecologists continued to be significant predictors. No interaction terms entered the model. ASSOCIATION BETWEEN PREDICTORS We evaluated associations between specific predictors: age younger than 45 years, nonwhite status, history of UTI, and visit to an obstetrician gynecologist. Younger and nonwhite patients were more likely to visit obstetricians gynecologists than the other primary care physicians OR, 4.18; 95% CI, 2.37-7.37, and OR, 1.71; 95% CI, 1.02-2.88, respectively ; . Nonwhites were younger than their white counterparts OR, 1.83; 95% CI, 1.14-2.93 ; . We found no other significant relationships. In particular, obstetricians gynecologists did not see patients with a history of UTI more frequently.

14 by document. It should be done as a matter of routine for all documents which are to be laminated or encapsulated; documents which are to be repaired by conventional hand repair methods should be tested individually for acidity e.g. with 'Mercx' strips ; and treated where necessary. 41. A number of alkaline chemicals are suitable for deacidification treatment: e.g. magnesium acetate, magnesium bicarbonate. These may be applied in various ways: by spraying, by damping with a sponge or by immersion. The latter is probably the most effective method. Aerosol deacidification sprays are available from conservation suppliers? these are of limited effectiveness, since they may not penetrate deep below the surface of the paper, but their use may be unavoidable where water soluble inks make the aquaeous methods unsuitable. Gaseous methods of deacidification should not be used without expert advice on the potential health hazards they may entail. Binding 42. Like traditional hand repair binding is a craft method which requires high levels of skill acquired over a lengthy period of practical training one year minimum ; . Productivity is low and craft binding is justified only where volumes with fine bindings require repair. Securing loose papers 43. Since traditional binding is not justified as a means of making up loose papers into assemblies and since modern commercial methods of binding are not suitable for archival use, alternative methods of securing loose papers have to be found. 44. The best method is guarding and filing, in which a paper-guard is either pasted to the edge of a sheet of paper or incorporated with it in the lamination process and a number of documents so treated are laced through holes drilled in the guards between either manila covers or boards. An alternative is to punch a hole in the margin.of the document itself and thread a number of documents on a 'treasury tag1 between manila covers. 'Treasury tags' are short lengths of cord with metallic or plastic ends secured so as to form a 'T'. Tags with corrodible metallic ends should not be used for archival purposes. Metallic pillar bindings should not be used. Electrostatic copying 45. In addition to copying documents by microfilming it is necessary to provide for some means of copying extracts from documents direct from the originals. This calls for an electrostatic copier with a flat non-moving platten. Ideally this should be capable of copying up to A3 size. Care should be taken, however, in using this process and, where documents are in a fragile condition or would for any other reason be damaged by copying in this way, microfilming and printing from the film should be preferred. Conclusions 46. A conservation and reprographic programme which strikes a balance between the best and the most economical methods and techniques should be developed as a matter of urgency. A list of the equipment and materials which would be required to establish such a programme is included in Appendix F. The staffing implications are considered in Chapter 6. A suggested layout for the Technical Services Room G 12 ; is included in Appendix B.

Prior to European contact, the tribe exercised regional political sovereignty. 1937 In December of 1936, The Tohono O'odham then known as the Papagos ; submitted a tribal constitution under provisions of the 1934 Indian Reorganization Act that was approved by the Secretary of Interior in 1937. That action resulted in the Papago Tribe being formally recognized by the U. S. Government. 1986 The Tohono O'odham revised their constitution. The revised constitution gives districts on the reservation more ability to enact their own ordinances but strengthens the power of the central legislative council on national issues. It also officially changed the name of the tribe to Tohono O'odham Nation, the traditional name of the people who had been given the name Papagos by early Europeans and pindolol.

No. 9 Cienega Creek Headcut Study Claire Zucker explained that this unique study is being funded by a grant from ADEQ. PAG is studying an erosional feature headcut ; in Cienega Creek and is documenting the hydrologic and habitat conditions as the headcut migrates upstream. The headcut is 16 feet high in spots causing trees to be uprooted and the banks to become unstable. As the water levels decline upstream from the headcut vegetation will die off in this critical riparian and aquatic habitat. Mead Mier, Watershed planner, described the tasks timeline for the project and provided an overview of well installation on the creek. A report will be published, January 2010. 3.
At the beginning of an experiment, all the weights of the network are initialized randomly. The robot is placed in the environment in which it runs around and learns the receptive fields of the different cells. For the sake of simplicity, learning is not engaged in all the layers simultaneously, but rather following a schedule which is given by the hierarchical structure of the network. Thus, initially, cell-groups within the first layer learn their receptive fields on the sensory inputs. It's not until the learning process has come to a stable state, i.e. the objective function O does no longer change over time, that learning is engaged in the subsequent layer. The whole learning process lasted approximately 3 hours. In order to locate the receptive fields of the cells in the highest layer of the network within the environment, the position of the robot as well as its orientation are tracked with a camera. After learning, the activity of the cells at the highest level of the network are recorded over time. Both, the position as well as the orientation at which the cells respond maximally are determined and plotted in Fig. 9.2. Most of the cells are clustered around a region in the lower left corner of the environment. The reason for this accumulation is not clear, only that it is concordant with the position of the light source which was placed outside the arena at this corner. Furthermore, the rest of the cells appear to have a tendency to distribute along the borders of the environment, while there are only a few located in the center. The typical selectivity of the cells with respect to the robot's position and orientation is shown in Fig. 9.3a & b. The response of the cell shown in Fig. 9.3a is particularly orientation specific while it is not very selective for the position of the robot. This cell has a strong affinity for the large red object in the upper-left corner of the environment see Fig. 9.2 ; , i.e. the cell only responds strongly if this object is within its visual field. The other cell, however, is more specialized for a particular position of the robot while being more tolerant with respect to its orientation Fig. 9.3b and pram.

With the exception of trastuzumab, which is now registered for use in patients whose tumours overexpress HER-2 as detected by the Herceptest, we do not yet have sufficient evidence to recommend the use of HER-2, p53 or BCL-2 in the selection of therapies for breast cancer. Further studies need to be performed. We will eventually need level 1 studies to confirm the predictive value of proposed molecular markers, as metaanalyses will be almost certainly be invalidated by editorial and reporting bias. Unlike clinical trials, which are usually eventually reported even if negative, companion studies are easily left unpublished if no significant result is demonstrated. There is now sufficient level 2 and 3 evidence that HER-2 may be predictive of resistance to hormonal therapy to justify a level 1 trial. Such a trial will be difficult to design however, as HER-2 + ER + patients are uncommon, and hormonal therapy versus no treatment placebo would not be ethical in the metastatic setting. As chemotherapy is rarely combined with hormonal therapy in metastatic disease, and an active agent would need to be given on each arm, the only possibility in metastatic disease would be a randomisation of HER-2 + ER + patients between an anti-HER-2 therapy, such as herceptin, given as a single agent versus the same agent in combination with hormonal therapy. Ideally, both sequential therapy and concurrent administration of the two agents would be investigated. Such a design demands the rather large assumption that no interaction exists between the two therapies. The trial would need to be powered as for equivalence, and it would be imperative to document PR and EGFR status. In the adjuvant setting, a design of chemotherapy alone versus.
038-inch guide wire. The sheath is advanced into the distal abdominal aorta. Once the sheath is in place, the inner cannula is removed, and the 0.063-inch guide wire is advanced into the abdominal aorta to the level of the diaphragm. The guiding catheter is then advanced over the guide wire through the sheath and into the abdominal aorta. The guide wire is removed, and the guiding catheter is used to select the renal artery orifice. The small coaxial catheter is then passed through the guiding catheter and across the stenosis. If the stenosis is not too tight, the coaxial balloon catheter can be advanced across the stenosis without the guide wire while a small amount of contrast material is injected. In the presence of a tortuous renal vessel, a tight stenosis, or a stenosis in a branch, the fine steerable guide wires greatly facilitate passage of the balloon catheter. The advantage of this technique is that the fine coaxial catheter passes through the tight stenoses more readily than the 7-F balloon catheter. It is also easy to steer this catheter out into the branches of the renal artery, especially over the 0.014-0.018-inch steerable guide wires. The small diameter of the balloon makes it possible to obtain pressures across the stenosis. The progress of the angioplasty can also be monitored by injecting contrast material either through the balloon catheter or and praziquantel.

EDITORIAL we identify features of the environment of the Mexican Pima Indians that might be helpful now to those populations who have progressed beyond the early phase of environmental change? This highlights the importance of scientific study in characterizing the specific environmental factors that relate to obesity development and translating that research into workable solutions that can be used in different cultures. Continued study of this population provides a wonderful opportunity for systematic learning. Most people in the United States live in an environment more similar to that of the Arizonan Pima Indians than of the Mexican Pima Indians. In this environment, populations not necessarily thought to have a thrifty gene are experiencing obesity in epidemic proportions. Although advances in genetics will almost certainly further our understanding of obesity and will provide tools to help prevent and treat obesity, without substantial changes in the environment, can we avoid suffering the same fate as the Arizonan Pima Indians? and prevnar.
Figure 3. Relative risks and 95% CI for all-cause mortality of men versus women in various patient groups. Relative risks are adjusted for the variables listed in Table 2 . The risk of death for men with nonischemic causes of heart failure is significantly greater than women with similar causes. In contrast, there was no significant increase in the relative risk of death for men versus women with an ischemic cause of heart failure.
Modi, K. N., Vartak, M. N., Shah, N. N., Lotlikar, K., and Sheth, U. K.: A Sequential Plan for Screening Compounds with Potent Diuretic Activity in Rats. Arch. Int. Pharmacodyn. 144: 51, July ; , 1963. The authors describe a sequential plan for the rapid screening in rats of orally active diuretic compounds, in doses that produce optimal responses. They outline a statistical method aimed at determining the optimal dose. The conventional arbitrary selection of a range with two or three doses for screening is likely to miss a "good" compound, if its optimal responsive dose is above or below this particular range. Various known diuretic drugs and new synthetic compounds derived from triazine, triazole, and bisulfonyl urea were screened by this method, and their potency was readily assessed. At least one compound from each of these three groups was found to be a more effective diuretic agent in the rats than was hydrochlorothiazide. MAuRSHALL and prialt.

Secretion of progesterone itself Li et ai, 1992; Rabinovici et ai, 1992; Cataldo et ai, 1994 ; . We have recently shown that activin stimulates the expression of inhibin pB-subunit mRNA in human granulosa-luteal cells but inhibits gonadotrophin-stimulated inhibin a-subunit mRNA Eramaa etal., 1995 ; . Human granulosa cells also produce the activin-binding protein follistatin Shimasaki et ai, 1988; Roberts et ai, 1993; Tuuri et ai, 1994 ; which appears to counteract the effects of activin in cultured granulosa-luteal cells Cataldo et ai, 1994; Eramaa et ai, 1995 ; . As inhibins and activins play important, and in many cases opposite, roles in the regulation of human ovarian function, we have set out to determine how ovarian inhibin activin subunits are regulated in cultured human granulosa-luteal cells. We previously reported that FSH and human chorionic gonadotrophin HCG ; induce inhibin a- and pA-subunit mRNA in these cells with different kinetics Eramaa et ai, 1994 ; . The basal inhibin pB-subunit transcript levels are very low in these cells Eramaa et ai, 1995; Eramaa and Ritvos, 1996 ; . Although gonadotrophins do not stimulate pB-subunit transcript levels, they are markedly induced by activin Eramaa et ai, 1995 ; and transforming growth factor TGF ; -p Eramaa and Ritvos, 1996 ; . The present study was undertaken to investigate whether the eicosanoids prostaglandins PG ; E2 and PGF 2a are involved in the regulation of granulosa cell inhibin mRNA and inhibin A secretion. PGE2 and PGF 2a have been found in significant amounts in human follicular fluid Seibel et ai, 815 and primaquine.
